Intake Forms
Families moving forward with services will be asked to complete an intake form to officially begin the counseling process.
Making an Appointment
To schedule an appointment, please contact our office at 585-264-0604.
Office hours: 10:00am - 2:00pm
Appointments are available at our office, with after-school and evening hours offered as needed.
Payment for Services
Office visits are $75 per session and are expected to be paid at the time services are provided. Payment plans are available based on financial need.
Families approved for services may submit payment using the link below. For questions about fees or need to discuss a payment arrangement, please contact our office. We are happy to work with you.
